Abstract : 

Chronic illnesses are a major challenge for healthcare systems today. This statement seems to be a mantra of recent times, which, in turn, is becoming a premise that is just as chronic, as the subject has been discussed for many years, with the resulting increase of these illnesses for the sustainability of the healthcare systems in developed countries today. Undoubtedly, data show that this challenge has already become a reality. Chronic illnesses represent the main epidemiological pattern, as they currently involve 80% of primary care consultations and 60% of hospital stays in Spain, representing more than 80% of healthcare expenditure.
